I am trying to attach an existing application to the user block but when hitting a submit button it is not working.
Any help would be helpful.

Apologies for that issue. I just looked into 'user tasks' and it seems that it is a private beta that has now ended, so it might be tricky for me to get access to this feature to test things out and try to reproduce that.
If your team/org has a TAM or anyone from the workflows team that was assigned to help and get feedback on this beta feature I would highly recommend forwarding this information along to them!
I can also send this feedback to our workflows team but I am not sure if they will be updating this feature until the next step of the product's beta comes out.

I just heard back from the Workflows team, they said that:
User tasks are not compatible with multi page apps, which is what they are probably experiencing. They can use "user actions" to connect user tasks to multipage apps instead.
Here is a loom recording of my team mate reproducing the behavior you are likely seeing and then the work around for setting things up with "user actions"!
