Agent chat/call frequently ends with 'Something went wrong'

Hi everyone,

I recently build up an agent on Retool with Claude Sonnet 4.5 as the base model, the API I was using was my own. Sometimes I receive this orange block message saying ‘Something is wrong‘, which always leave me no choie but to re-run the agent. Sometimes a re-run will solve the problem, but this is certainly frustrating and definitely something I don’t want my users to experience.
When I check the log, everything(tools, agentruns) related to that stopped run is labelled as ‘Failed’.
What is the source of this problem, and how can I fix it?

I looked into the Dec 15th logs for your org, but, unfortunately, I couldn't track down a more descriptive error message.

Has this happened again? If so, could you please provide a more recent date/time/timezone for us to investigate in the logs?
