Data from a new Form Application Altered with Change of Resource Table Name

We have a Form App that I changed the Table Name. This caused the Form App to fail. Simply changing the resource to the new name did not work. Then changing the table name back to it original name worked but the data is partially lost.

This was a standalone form.

I reproduced your error using RetoolDB and was able to resolve the issue by reconnecting the resource, clicking "Generate form" and then clicking publish and update form. Have you tried those steps?
