Any MySQL resource that has the flags for "Use dynamic database names" or "Use dynamic database host" do not have the ability to change or see those fields unless you disable and then reenable those flags. You have to do this workaround each time you reload the page.

Filling this ticket for you now, apologies for the delay.
I remember testing this out and confirming I could reproduce the bug with you during office hours but forgot the exact steps to reproduce this 
I remember seeing the input fields in the query disappear when they shouldn't but can't recall when caused that to happen.
I do remember that we needed to go into the resource set up and toggle those two check boxes off then on again to get the input fields in the query to re-appear. If you could shoot me those steps that would be amazing!
